Your Specific Responsibilities:
This is a professional position within the St. Lucie County Health Department utilizing established skills in the assessment, planning, organizing, delivering, and evaluating of services provided to non-pregnant sexually active adolescents in the Teen Choices program. Responsibilities include educating sexually active adolescents about sexual health, preconception health, HIV/STD prevention, reproductive life planning and to decrease the unintended birth rates and infection rates in this population. This position requires the employee to work in a location geographically away from direct supervision. Supervision is provided by the Community Health Nursing Consultant. Employee is expected to drive to other locations to conduct the duties outlined here. Use of personal vehicle may be required.
The program objectives are as follows:
1. Decrease the number of repeat pregnancies within the teen mothers who are being case managed by increasing the number of teen mothers who use birth control after they deliver their babies. 80% of this group will use birth control after they deliver. Avoidance of a repeat pregnancy will be considered successful if the teen is not pregnant within two years after her first delivery.
2. Achieve 90% of non-pregnancy within the sexually active teens who are case managed for at least 3 months.
3. Unintended pregnancy will be avoided by 90% of the sexually active teen population that participate and complete the Teen Choices one year after having completed the curriculum.
4. 90% of teenagers who are receiving one on one case management will demonstrate an increased awareness regarding vocational and educational resources by discussing career and school opportunities as measured by progress or written case plan.
Group education:
Instructs adolescents in varied group settings on issues related to health education, family-life skills, postponing sexual activity, family planning methods, sexually transmitted diseases, HIV & STD, parenting skills, employment skills, setting boundaries, healthy relationships, alcohol & drug use, being able to find a trusted adult to talk to, male & female sexual anatomy & physiology, reproductive life planning, preconception health, goal setting, time management & decision making skills, negotiation & refusal skills, educational resources (including high school, GED, college programs), personal hygiene, dental care, and the REAL Essentials Starting Points curriculum.
Community engagement:
Develop partnerships and collaborate with all adolescent-focused community-based projects and programs providing services to adolescents including, but not limited to the Parent Teen Community Connection, Teen Pregnancy Prevention Network, Teen Zone, Florida Community Health Centers, and the Department of Health. Enroll new groups to the program. Represents the Department of Health at local meetings as assigned by the supervisor. Assigned to Teen Zone clinic once every 3rd-4th clinic under the direction of the Teen Zone Coordinator.
Program Promotion:
Coordinate outreach events.
Participate in at least 4 health fair/fundraising events per fiscal year.
Conduct outreach/groups to positively promote the program at various forums.
Enters EARS daily and certifies them within 7 days from the date of service. Enters CSR’s daily. Collect weekly stats to conduct monthly and quarterly reports. Maintain sign in sheets for each class, as well as pre/posttests. Maintain logs to ensure appropriate follow up is done on students one year after completing the Teen Choices curriculum.
